 New Student Pre-Registration

Do you have a student entering Uintah for the first time for school year 2025-2026? Please PRE-REGISTER as a NEW STUDENT so that we have all of your contact information:

 Also, please bring the following required information to the office:

• Original birth certificate (No copies will be accepted, we will make a copy)
• Current immunization records 
• 2 proofs of residency
• We will take a copy of parents identification

• Be sure to select new student    
• BE SURE TO SELECT SCHOOL YEAR 25-26
• Be sure to complete ALL fields under ALL tabs
